Carlo Urbani – microbiologist

The doctor and microbiologist Carlo Urbani, whose decisive action after discovering the deadly SARS virus saved millions of lives, was born on this day in 1956 in Castelplanio, near Ancona.

Dr Urbani himself died after contracting the condition, which had been given the name severe acute respiratory syndrome.

He identified it in an American businessman who had been taken ill in Hanoi, the capital of Vietnam, with suspected influenza.

Recognising quickly that what he was dealing with was not a straightforward case of ‘flu, Urbani, who was working in Vietnam as an infectious diseases specialist for the World Health Organisation, immediately alerted WHO headquarters in Geneva.

He convinced them that what he had discovered posed a grave threat to life and thus sparked the most effective response to a major epidemic in the history of medicine.

At a local level, be persuaded the Vietnamese health authorities to introduce a raft of preventative measures, including large-scale screening and prompt, secure isolation of suspected victims, that slowed the spread of the disease.

It was as a result of Urbani’s actions that the epidemic was largely contained, almost all deaths from the disease occurring in territories surrounding the South China Sea, namely southeast China, Hong Kong, Vietnam, Singapore and the Philippines.

Some 775 people died in the epidemic, two thirds of them prior to Urbani’s intervention.

The only significant outbreak beyond this area was in Canada, where 44 people died following the import of the disease into the country by a tourist who had stayed at the same hotel in Hong Kong as Urbani’s patient, who probably contracted the disease from a Chinese doctor who was a guest at a wedding there. 

This doctor had been treating patients suffering from the disease in Guangdong Province, just across the border from Hong Kong, where the virus is thought to have originated.

Urbani, whose parents were a teacher at the Ancona Commercial Navy Institute and a primary school headmistress, gained a degree in medicine from the University of Ancona and a postgraduate degree in tropical parasitology from the University of Messina in Sicily.  He began voluntary work in Africa while he was still at university.

After a period of academic research, he went to work at a hospital in Macerata, about 55km (34 miles) from his home town of Castelplanio in the Marche region.  In 1993 he began to accept temporary assignments from the WHO in places such as the Maldives, Mauritania and Guinea.

In 1997, on a recommendation from the WHO, he joined Médicins San Frontières to work in Cambodia and Vietnam. He was elected president of the Italian section of MSF in 1999 and travelled to Oslo in the same year to receive the Nobel Peace Prize on behalf of the organisation.

A year later he was recruited by the WHO to be their infectious diseases specialist in Vietnam, Cambodia and Laos, based in Hanoi. He achieved success in reducing the health impact of parasitic flatworms.

Urbani was summoned to Hanoi’s French Hospital following the admission on February 23, 2003 of a 47-year old Chinese-American businessman called Johnny Chen, resident in Shanghai, who had begun to feel unwell with 'flu-like symptoms soon after arriving on a flight from Hong Kong.

He became suspicious that Mr Chen had something different from normal influenza when several hospital workers who had been in contact with him quickly developed similar symptoms, some rapidly becoming seriously ill.

Sadly, the story has a tragic postscript.  After treating a number of sick patients, Urbani left Hanoi on March 11, bound for a conference in Bangkok, Thailand, where he was to give a talk on the subject of childhood parasites.

During the flight he became feverish. He realised straightaway what had happened and when he arrived at the airport in Bangkok told a colleague sent to meet him that he should call an ambulance immediately and inform the operator that he had almost certainly contracted the SARS virus.  While they awaited the ambulance, Urbani insisted his to his colleague that he was to allow no one within eight feet of him.

Urbani was in intensive care for 18 days but died on March 29, having ultimately suffered a heart attack.  He was only 46 and left behind a wife and three children.

The walled city of Macerata in Marche, where Carlo Urbani worked in the local hospital, is a charming historic city of cobbled streets perched on a hill between the Potenza and Chienti rivers.  At the heart of the city, in the pretty Piazza della Libertà, is the Loggia dei Mercanti with its two-tier arcades, dating from the Renaissance. Macerata’s university is among the oldest it Italy, established in 1290.  Each July and August the city hosts the Sferisterio Opera Festival, which is held in the 2,500 seat open-air Arena Sferisterio, a huge neoclassical arena built in the 1820s.

The coastal city of Ancona, where Urbani attended university, is a bustling port with a population of almost 102,000. Although the area around the port has an industrial feel, there are some notable beaches nearby and a good deal of history in the older part of the city, bearing witness to its Greek and Roman past. The 18m-high Arch of Trajan, built in honour of the emperor who built the city’s harbour, is regarded as one of the finest Roman monuments in the Marche region. Urbani may have been inspired to follow his chosen direction in life by the presence in Ancona’s harbour of the Lazzaretto, the pentagonal building constructed on an artificial island in the 18th century as a quarantine station designed to protect Ancona from diseases carried by infected travellers.

Gabriele Falloppio, one of the most important physicians and anatomists of the 16th century, died on this day in 1562 in Padua.

Often known by his Latin name Fallopius, he lived only 39 years yet made his mark with a series of discoveries that expanded medical knowledge significantly.

He worked mainly on the anatomy of the head and the reproductive organs in both sexes and is best known for identifying the tubes that connect the ovaries to the uterus, which are known even today as Fallopian tubes.

He also discovered several major nerves of the head and face, and identified many of the components of the hearing and balance systems.

Falloppio described all of the findings of his research in a book published a year before he died, entitled Observationes anatomicae.

Educated initially in the classics, the death of his father plunged his family – noble but not wealthy – into financial difficulties, prompting him to pursue the security of a career in the church, becoming a priest in 1542. He served as a canon at the cathedral in his native Modena.

Falloppio retained an ambition to study medicine, however, and when the family’s finances had improved sufficiently he enrolled at the University of Ferrara, which at the time had one of the best medical schools in Europe.

He studied under Antonio Musa Brassavolo, who at the time was one of the most eminent physicians in Europe, with a list of illustrious clients that included King Henry VIII of England, Holy Roman Emperor Charles V, the French king Francis I and a succession of popes.

After receiving his doctorate in medicine, he worked at various medical schools before becoming professor of anatomy at Ferrara in 1548.  A year later, he was invited to occupy the chair of anatomy at the University of Pisa.

Falloppio gained much of his knowledge from dissecting cadavars, not only those of adult humans but children and animals.  During his time at Pisa he was falsely accused of human vivisection, but despite the cloud this cast over him he was appointed to the prestigious chair of anatomy at the University of Padua, where he would remain until his death from tuberculosis.

The Flemish anatomist Andreas Vesalius was among his predecessors in the Padua chair.  It was the work of Vesalius that prompted a surge of interest in dissections and probably inspired Falloppio, who studied the observations of his predecessor in great detail and sought to build on them.

Despite his short working life, he left an enormous legacy of research.

He carried out investigations on the larynx and on respiration, and made important discoveries about bone growth. He described the ethmoid bone, the lacrimal duct, and his description of the middle and inner ear includes the first clear account of the round and oval windows, the cochlea, the semi-circular canals, and the scala vestibuli and tympani.

In the area of reproduction, as well as being the first to identify the Fallopian tubes, he proved the existence of the hymen in virgins, gave names to many features of the reproductive anatomy and disproved many popular notions about the mechanics of the reproductive process.

He can also be credited with inventing one of the earliest condoms, a sheath made from linen soaked in a medicinal chemical to be worn to protect the wearer from contracting syphilis.

Falloppio published two treatises on ulcers and tumors, a treatise on surgery, and a commentary on Hippocrates's book on wounds of the head.  He also researched the science of baths and thermal waters and of purgatives, and put forward important theories about the formation of fossils.

The University of Padua includes nine museums, a botanical garden – best visited in the spring and summer – and the oldest surviving permanent anatomical theatre in Europe, built in around 1595 and which used to hold public dissections, which attracted scientists and artists in large numbers, keen to enhance their knowledge of the human body.

The city of Padua, situated in the Veneto a 30-45 minute train ride from Venice and an hour and a half from the international airport at Treviso, is most famous for the Giotto frescoes at the Scrovegni Chapel and for the Basilica of St Anthony of Padua. Both attract thousands of visitors and the Scrovegni Chapel requires advance booking.  The city itself is an attractive place to explore, with a wealth of fine, historic buildings to discover along its pleasant arcaded streets, as well as the beautiful Prato della Valle, the 90,000-square metre elliptical square with an island in its centre surrounded by a canal bordered by 78 statues.

Roberto Assagioli – psychiatrist

Harsh imprisonment sparked new psychiatric theories


Roberto Assagioli, the pioneering psychiatrist who founded the science of psychosynthesis, died on this day in 1974 in Capolona in the province of Arezzo in Tuscany.

His innovative psychological movement, which emphasised the possibility of progressive integration, or synthesis, of the personality, aimed at finding inner peace and harmony. It is still admired and is being developed by therapists and psychologists today.

Assagioli explained his ideas in four books - two published posthumously - and the many different pamphlets he wrote during his lifetime.

In 1940 the psychiatrist had to spend 27 days in solitary confinement in prison, having been arrested by Mussolini’s Fascist government for praying for peace and encouraging others to join him. He later claimed this experience helped him make his psychological discovery.

Assagioli was born under the name of Roberto Marco Grego in 1888 into a middle-class, Jewish background in Venice.

His father died when he was two years old and his mother remarried quickly to Alessandro Emanuele Assagioli. As a young child Roberto was exposed to art and music and learnt many different languages – creative inspiration which is believed to have helped his work in psychosynthesis.

When he was 18, Assagioli began to travel, and while in Russia he learnt about social systems and politics.

Assagioli received his first degree in neurology and psychiatry at Istituto di Studii Superiori Pratici di Perfezionamento in Florence in 1910. Then he began writing articles that criticised psychoanalysis, arguing for a more holistic approach.

After training in psychiatry at a hospital in Zurich, he opened the first psychoanalytic hospital in Italy, but felt unsatisfied with this field of psychiatry.

He married Nella Ciapetti in 1922 and they had a son, Ilario.

After being released from his solitary cell in Regina Coeli prison in 1940, he returned to his family, but later in the war their farm was destroyed and they had to go into hiding in the mountains above Arezzo.

Their son, Ilario, died at the age of 28 from lung disease, thought to have been caused by the harsh living conditions he experienced during the war.

After the war, Assagioli returned to his work on psychosynthesis, preferring a spiritual and holistic approach to psychology.

He was inspired by Freud and Jung and felt that love, wisdom and creativity were important components in psychoanalysis.

Assagioli corresponded with Freud but they never had the chance to meet.

He gave much of the credit for his inspiration for psychosynthesis to his solitary confinement for nearly four weeks in 1940.

He said he used his time in prison to exercise his mental will by meditating daily because he had realised he was able to change his punishment into an opportunity to investigate his inner self.

Assagioli died on August 23, 1974 at the age of 86 from unknown causes.

Since his death, psychosynthesis has continued to be embraced as a comprehensive psychological approach for finding inner peace and harmony.

Roberto Assagioli’s former home in Via San Domenico, on the northern outskirts of Florence on the way to Fiesole, is now the headquarters of the Institute of Psychosynthesis, where conferences about the science are regularly held. Although Assagioli wrote just two books about his ideas, the Institute houses a rich archive of documents that includes a large quantity of hand-written material by him.

Capolona, where Assagioli was living when he died, is a small town near Arezzo in Tuscany, located on the right bank of the River Arno. It is referred to as the gateway to the Casentino area, which is rich in castles, churches and old bridges. A few kilometres away to the north lies Caprese Michelangelo, the small village where the great artist Michelangelo was born.

Vittorio Erspamer, the pharmacologist and chemist who first identified the neurotransmitter serotonin, was born on this day in 1909 in in the small village of Val di Non in Malosco, a municipality of Trentino.

Serotonin, also known as 5-hydroxytryptamine (5-HT), is found in the gastrointestinal tract, blood platelets and central nervous system of animals, including humans.

It is popularly thought to be a contributor to feelings of well-being and happiness. A generation of anti-depressant drugs, including Prozac, Seroxat, Zoloft and Celexa, have been developed with the aim of interfering with the action of serotonin in the body in a way that boosts such feelings.

The name serotonin was coined in the United States in 1948 after research doctors at the Cleveland Clinic in Ohio discovered a vasoconstrictor substance - one that narrows blood vessels - in blood serum. Since it was a serum agent affecting vascular tone, they named it serotonin.

However, in 1952 it was shown that a substance identified by Dr Erspamer in 1935, which he named enteramine, was the same as serotonin.

Dr Erspamer made his discovery when he was working as assistant professor in anatomy and physiology at the University of Pavia, having graduated there in medicine and surgery in 1935.

His speciality was pharmacognosy - the study of drugs from natural sources. In particular, he was interested in the extraction of pharmacologically active substances from animals, which was the focus of much of his life’s work.

Dr Erspamer’s early research in the Institute of Comparative Anatomy and Physiology at Pavia focussed on the smooth muscle constricting or contracting properties of various compounds – known as amine - found in the skins and intestinal tracts of a number of species, including rabbits, mollusks, and frogs.

One substance which interested him was found in certain cells of the gut. An acetone isolate of the cells caused smooth muscle contraction, especially in the uterus of the female rat.

He carried out tests to prove that the substance was not another neurotransmitter, epinephrine - also known as adrenaline – and named the new substance enteramine.

During his career, Dr Erspamer held positions at the universities of Rome, Bari and Parma and also studied in Berlin.

He was one of the first Italian pharmacologists to realize that strong relationships with the chemical and pharmaceutical industries could yield vital funds for research.

In the late 1950s, he established a long-term collaboration with chemists at the Farmitalia company, thanks to whose funding he collected more than 500 species of marine organisms from all around the world, including amphibians, shellfish, sea anemones and other species.

During more than 60 years he was able to conclude the isolation, identification, synthesis and pharmacological study of more than 60 new chemical compounds, most of which were isolated from animals, predominantly amphibians.

His other major discovery was octopamine, a substance similar in function to epinephrine in that it mobilises the body and the nervous system for action. He found this in the salivary glands of the octopus.

Twice nominated for a Nobel Prize, he was obliged to retire from official academic positions in 1984 on the grounds of age but continued to work at the Sapienza University of Rome, alongside his wife Giuliana Falconieri, a long-time colleague he married in the early 1960s, up until the time of his death in 1999 at the age of 90.

Vittorio Erspamer’s birthplace, Malosco, is a small village in the upper Val di Non valley situated on a high plateau about 54km (34 miles) north of Trento in an area of forests and meadows. The discovery of coins in the vicinity points to Roman origins. More recently, it belonged to the family of count Gerolamo Guarienti, who rebuilt Malosco Castle in the 16th century. Today it is a popular centre for cross-country skiing and there is a network of trails for walkers to enjoy.

Although not established until 1361 – almost 300 years after the University of Bologna, which is recognised as the oldest in Europe – the University of Pavia can claim to have its roots in an educational institution in the Lombardy city of which the first known mention was in 825, in an edict issued by the Frankish king of Italy, Lothar I. That would make it older even than Al Quaraouyine University, in Morocco, which was founded in 859 and is officially the oldest continually operating educational institution in the world.

Giuseppe Forlenza, an important 18th century ophthalmologist and surgeon, was born on this day in 1757 in Picerno in the province of Potenza.

He became famous for performing successful cataract surgery and for his treatment of eye diseases.

Forlenza was born in the region of Basilicata, which at that time was part of the Kingdom of Naples. His father and two uncles were all surgeons.

He went to Naples and then on to France to study surgery. He spent two years gaining experience at St George’s Hospital in London and then returned to France where he concentrated on treating eye diseases.

Forlenza carried out eye surgery at a retirement home in Paris and performed many remarkable operations on soldiers returning from fighting in Egypt who were suffering from eye problems.

He was recognised as a leading eye surgeon by Napoleon, who in a royal decree assigned him to treat eye disease throughout France.

Forlenza eventually returned to Italy where he performed many free operations in Turin and Rome and he wrote what was considered at the time to be a forward thinking medical work about the treatment of eye disease.

Forlenza died in 1833 in Paris at the age of 76.

Potenza is the capital of the Basilicata region in southern Italy, built on high ground overlooking a valley in the Apennine mountains of Lucania. A tower from an old castle and three gates from the former city walls still remain and there are the ruins of a Roman villa to see.

The University of Naples was founded in 1224 by the Emperor Frederick II. One of its most famous students was the theologian and philosopher Thomas Aquinas who went on to lecture there in the 13th century. A former college built in the 16th century in Via Paladino, in the area of Spaccanapoli, has been the main university building since 1777.

Antonio Maria Valsalva – anatomist

Work by brilliant professor benefits astronauts today

Antonio Maria Valsalva, a much respected anatomist, died on this day in 1723 in Bologna.

Valsalva’s research focused on the anatomy of the ear and his discoveries were so important that a piece of equipment used by astronauts today is named after him.

The Valsalva device in spacesuits allows astronauts to equalise the pressure in their ears by performing the Valsalva manoeuvre inside the suit without using their hands to block their nose. It has also been used for other purposes, such as to remove moisture from the face.

Valsalva was born in Imola in 1666. He received an education in humanities, mathematics and natural sciences before going on to study medicine and philosophy at Bologna University. He later became Professor of Anatomy at Bologna University.

His main interest was the middle and inner ear and it was Valsalva who coined the term Eustachian tube for a part within the ear. It was named after the 16th century anatomist Bartolomeo Eustachi who had described the tube in his written work.

The Valsalva manoeuvre, the forcible exhalation against a closed airway, often practised by people to equalise pressure between the ears when on an aeroplane, is still used by doctors today to help them with diagnosis in certain situations.

Valsalva’s most substantial contribution to the field of medicine was his detailed, illustrated work about the anatomy, physiology and pathology of the ear, published in 1704.

Valsalva died after a stroke in 1723 and was buried in the church of San Giovanni in Monte in Bologna.

Imola, where Antonio Valsalva was born, is a City in the province of Bologna in Emilia-Romagna. It dates back to Roman times and there are many fascinating historic buildings to see in the centre. The castle, the Rocca Sforzesca, is well preserved, and is nowadays the home of an internationally respected piano academy and the Cinema d’Este, which shows films in July and August. Imola is also home to the Autodromo Internazionale Enzo e Dino Ferrari, a famous automobile racing circuit.

Bologna University was founded in 1088 and is the oldest in the world. There is a portrait of Valsalva in the university’s oldest surviving building, the Archiginnasio, which is now a library. It is open Monday to Friday from 9am to 7pm, and on Saturdays from 9am to 2pm and is just a short walk from Piazza Maggiore and the Basilica di San Petronio in the centre of the city.
